1.5-kg Fibroid Uterus Removed Through Keyhole Surgery at Thodupuzha Baby Memorial Hospital

Thodupuzha: At Baby Memorial Hospital, Thodupuzha, a Total Laparoscopic Hysterectomy (TLH) was successfully performed on a 46-year-old woman who experienced heavy uterine bleeding and severe anaemia due to an enlarged fibroid uterus. The patient, a resident of Madakathanam, Thodupuzha, had a haemoglobin level of 7.4 g/dL. Considering the severe anaemia caused by the bleeding, the patient was given a blood transfusion before the surgery.

On examination, the uterus was found to be the size of a 28-week pregnancy and contained 20 fibroids. The largest fibroid measured approximately 8 × 8 cm. The uterus along with the fibroids removed through surgery weighed approximately 1.5 kg. A Total Laparoscopic Hysterectomy was performed on October 7, 2026, through which the uterus and cervix were completely removed. The main highlight of this case was the successful completion of laparoscopic surgery on a uterus the size of a 28-week pregnancy, weighing 1.5 kg and containing 20 fibroids.

In the case of a uterus of such a large size, open surgery is generally considered. Laparoscopic surgery on a uterus of this size is technically very challenging. Therefore, successfully completing the surgery through a minimally invasive laparoscopic approach makes this case noteworthy. Laparoscopic surgery offers advantages such as reduced postoperative pain, smaller incisions, faster recovery and a shorter hospital stay. Following the surgery, the patient recovered well and was discharged after approximately three days of hospitalisation.

The surgery was led by Dr. Anooja Sebastian, Senior Consultant, Obstetrics & Gynaecology, along with Dr. Evelin. In the Department of Anaesthesia, Dr. Sunil A, Senior Consultant, and Dr. Nitish Roy, Associate Consultant, led the anaesthesia team. The nursing team included Jinto, Shaiju and Smitha.
